Mounds View Youth Hockey Association

Registration for the 2008-2009 season will begin on August 26th.  You can register in-person, online, or by mail.  Please be sure you complete all required forms.  The forms are below.  Your player's registration will NOT be complete until all required forms and payments have been received.  All forms and payment must be received before your player can participate in warm-ups or try-outs.  Try-outs start on September 27th.  Any registrations received after September 30th, will be assessed a $ 25 late fee.

If your player is new to MVYHA, you will need to provide us with a copy of their birth certificate.

Starting with the 2008-2009 season, Mounds View Youth Hockey will be implementing a new volunteer commitment policy.  As a part of your player’s registration, each family will need to complete a “Volunteer Commitment Registration Contract” and submit a separate check.  More information can be found under the “volunteers” tab above.

In-person registration will be held at the Shoreview Community Center on August 26th from 6:00-9:00 p.m. and September 4th from 6:00-8:00 p.m.  Registration forms are available below if you would like to complete them ahead of time.  Forms will also be available at registration.

Online Registration is available for Squirts, PeeWees, Bantams, U10, U12, U14 for the upcoming hockey season.  Note:  Initiation/Mite players may NOT register online; they must attend an "in person" registration day as listed above or register by mail. 

Once you've completed online registration, you must print and fill-out the forms found below (Consent to Treat/Medical History, Waiver of Liability, and Volunteer Commitment Registration Contract).  Mail the forms and and a check for the volunteer commitment contract to the address listed below. You may also bring the paperwork to the in-person registration at the Shoreview Community Center. Your player’s registration will NOT be complete until all of the required forms and payments have been received.

Please note that there is a fee to register online of approximately 3.25% of your total order, plus $ 1.50 per transaction.
